Required immunizations

Grades K-2

MMR (measles, mumps, rubella - two doses); Hepatitis B series (three doses over a six-month period); DTP (four or five doses); Polio (three or four doses); Varicella (chickenpox).

Grades 3-5

Same as shots required for grades K-2 but no Varicella requirement.

Grade 6

MMR (measles, mumps, rubella - two doses); DTP (four or five doses); Polio (three or four doses).

Grades 7-12

Same shots as required for grades 5-6 plus Tetanus-diphtheria (Td) booster; Hepatitis B series (two or three doses).
