New to the district? Register your child

Having the proper documents can make the process easier for you and your child.

The first step for families new to the district who want to apply to any Pinellas County school - choice attendance area or countywide magnet, fundamental or career academy - is registering with the school district.

Registration is a two-step process. The first step requires a visit to one of the district's two Family Education and Information Centers. The second step is completed at the school to which the student is assigned in the computer match process. If parents are registering after the application period has ended, the step is completed at the school to which a customer service representative at a center has assigned the child.

The most important point for families to remember as they begin the registration process is that they should come to the Family Education and Information Center with the necessary documents. Parents who arrive without the proper documents must come back with the paperwork.

Here is what you must provide to the customer service representative at the Family Education and Information Center to register your child:

Proof of birth, such as an original birth certificate; a certificate of baptism showing the date of birth and place of baptism, with an affidavit sworn to by the parent; an insurance policy on the child's life that has been in force for at least two years; a bona fide Bible record of the child's birth, with an affidavit sworn to by the parents; a passport or certificate of arrival in the United States showing the child's age; or a transcript record of age shown in the child's school record of at least four years before application stating the birth date.

Proof of residency, such as a utility bill for power, water, cable, sewer, land-based (not cellular) telephone, or rental agreement or lease, or closing document or tax statement with a homestead exemption with the parent or guardian's name and home address. Families who are staying with relatives or friends must obtain an affidavit of residency at the Family Education and Information Center, fill it out and have it notarized. It must be submitted to the customer service representative along with a lease or utility bill in the name of the person with whom the family is residing and who is listed on the affidavit.

Child's Social Security number. The district is required to ask for one, but the child is not required to have one.

Child's most recent report card if he or she is entering grades 1-12. It should include the school's address and phone number.

What you must take with you to the school to which your child is assigned to complete registration:

Florida certificate of immunization form DOH 680 appropriate for the child's grade level.

Physical examination certificate signed by a licensed examiner within 12 months before registration.

Child's most recent report card, if he or she is entering grades 1-12, that includes the school's name and phone number.
