Required immunizations

Grades K-4

MMR (measles, mumps, rubella - two doses); hepatitis B series (three doses over a six-month period); DTP (four or five doses); polio (three or four doses); varicella (chicken pox) or documented history of the illness.

Grades 5-6

Same as shots required for Grades K-4 but no varicella requirement.

Grades 7-12

Same shots as required for Grades 5-6 plus tetanus-diphtheria (Td) booster; hepatitis B series (two or three doses).

All immunization dates must be on the Florida Certificate of Immunization (DH 680).

All Pinellas County Health Department centers provide free immunizations.
