Fresh silverware.
If I pop five bucks more for a dessert, I want appropriate and clean utensils with it. That's common courtesy, not frilly French service. Telling me to keep my knife and fork once I've left them across the plate (a universal sign of course completion) is lazy, stingy and dirty. I'd forgive it in a diner, but most diners are too thoughtful and hospitable to suggest I use a dirty fork.
