Immunization Requirements

Students entering kindergarten through fourth grades must be current on: DPT (diphtheria, pertussis, tetanus); polio (IPV, OPV); MMR (measles, mumps, rubella); hepatitis B series (HBV); chicken pox (varicella) vaccine or have had the disease. Students entering fifth through sixth grade must be current on: DPT (diphtheria, pertussis, tetanus); polio (IPV, OPV); MMR (measles, mumps, rubella); hepatitis B series (HBV). Fifth-graders must also show proof of having had chicken pox or a record of having received varicella immunization. Students entering seventh through 12th grades: DPT (diphtheria, pertussis, tetanus); polio (IPV, OPV); MMR (measles, mumps, rubella); hepatitis B series (HBV); Td booster (tetanus, diphtheria). Contact your personal physician or the Hillsborough County Health Department, where immunizations are provided free to children who do not have a personal physician.
