Documents needed include: 1. A report card or a transcript copy from the student's last school. 2. Address verification from one of the following: a current electric bill, tax receipt, homestead exemption card, home purchase contract or a warranty deed. 3. Immunization records showing proper vaccines. 4. An authenticated record of the student's birth date. Students beginning school or students coming from outside Florida or a private school need to have a record of a physical examination from a private physician or the Hillsborough County Health Department within the past 12 months. A parent must be present at registration.
