Immunizations

  • KINDERGARTEN: Measles vaccine (second dose, preferably MMR); hepatitis B series (three doses over a six-month period); DTP (total of four or five doses required); polio (total of three or four doses required), varicella (chicken pox).
  • GRADES 1-3: Shots required for kindergarten but not varicella requirement
  • GRADES 4-6: Measles vaccine (second dose preferably MMR); DTP (total of four or five doses required); polio (total of three or four doses required)
  • GRADES 7-11: Shots required for grades 4-6 plus tetanus-diphtheria (Td) booster: hepatitis B series (two or three doses). Check with physician, if you're interested in a two-dose vaccine for children 11-15
  • GRADE 12: Measles vaccine; DTP (total of four or five doses required); polio (total of three or four doses required.

All immunization dates must be on the Florida Certificate of Immunization (DH 680). For immunization information, contact your private physician or one of the Pinellas County public health units below:
