1. BEARSS AVENUE AT LIVINGSTON AVENUE, SEPT. 27, 6:45 A.M.: A westbound 1994 Ford slid on the wet road and rear-ended a 2000 Chevrolet truck stopped for a red light. The Ford driver, a 38-year-old woman, was taken to University Community Hospital and charged with careless driving, expired license and tag.

2. GUNN HIGHWAY AT LUTZ LAKE FERN ROAD, SEPT. 28, 3:50 A.M.: A northbound 2000 Mazda was driving over the speed limit around a curve, lost control, went off the road, went into the air and landed upside down in a tree 25 feet from the road. The driver, a 36-year-old man, refused medical treatment.

3. MORRIS BRIDGE ROAD AT INTERSTATE 75, SEPT. 28, 4:30 A.M.: An eastbound 1995 Mitsubishi crossed over a double yellow line into the path of a westbound 1994 Honda motorcycle. The motorcyclist, a 72-year-old man, was ejected. The Mitsubishi spun around and came to a stop in the grass. The injured were taken to Tampa General and University Community hospitals.

4. INTERSTATE 275 AT FLETCHER AVENUE, SEPT. 28, 7:20 A.M.: A southbound 1997 Ford truck rear-ended a 1993 Chevrolet truck. The Ford continued and sideswiped a concrete barrier wall. The Ford driver, a 47-year-old woman, said she was taking over-the-counter medication for migraines. She was charged with careless driving.

5. HILLSBOROUGH AVENUE AT MEMORIAL HIGHWAY, SEPT. 28, 11:42 A.M.: A westbound 2000 Dodge hit a curb in the drive-through of a Dunkin' Donuts. The vehicle did not have enough room to get through due to construction and barricades.

6. FLORIDA AVENUE AT FLETCHER AVENUE, SEPT. 28, 12:42 P.M.: An unknown vehicle pulled out of a bank parking lot into the path of a southbound 2000 Plymouth. The unknown vehicle left the scene.

7. DALE MABRY HIGHWAY AT NORTH LAKEVIEW DRIVE, SEPT. 28, 1:05 P.M.: A southbound 2002 Chevrolet truck rear-ended a 1990 Toyota, pushing the Toyota into the rear of a 1991 Chevrolet van. The 2002 Chevrolet driver, a 27-year-old man, was charged with careless driving and no proof of insurance.

8. BEARSS AVENUE AT PINECREST ROAD, SEPT. 28, 3:15 P.M.: A southbound 1986 Honda turned left into the path of an eastbound 2001 Oldsmobile. The Honda driver, an 83-year-old woman, was charged with violation of a traffic control device.

9. VAN DYKE ROAD AT DALE MABRY HIGHWAY, SEPT 28, 3:41 P.M.: A southbound 1994 Lexus turned left into the path of a westbound 2000 Nissan. The Lexus driver, a 45-year-old man, was charged with failure to yield to approaching traffic. The Nissan driver, a 20-year-old woman, suffered injuries to her right wrist and was taken to St. Joseph's Hospital.

10. VAN DYKE ROAD AT CALUSA TRACE BOULEVARD, SEPT. 28, 4:10 P.M.: An eastbound 1986 Toyota rear-ended a 1995 Chevrolet van. The Toyota driver, an 18-year-old woman, was taken to St. Joseph's Hospital with non-life threatening injuries.

11. FLETCHER AVENUE AT NEBRASKA AVENUE, SEPT. 28, 4:45 P.M.: An eastbound 1996 Chevrolet rear-ended a 1994 Nissan. The Chevrolet driver, a 53-year-old man, was taken to University Community Hospital with non-capacitating injuries and charged with careless driving.

12. HILLSBOROUGH AVENUE AT WEBB ROAD, SEPT 28, 4:55 P.M.: A westbound 1990 Mercury pulled out from a parking lot into the path of a northbound 1995 GMC truck. The Mercury driver, a 45-year-old man, was charged with violation of right of way. The GMC driver, a 59-year-old woman, was charged with no proof of insurance.

13. LIVINGSTON AVENUE AT SPRING GREEN DRIVE, SEPT 28, 10:13 P.M.: A northbound 1995 Mitsubishi pulled off the road and made a U-turn into the path of a northbound 1996 Toyota truck. The Toyota left 65 feet of skidmarks. The Mitsubishi driver, a 22-year-old woman, was charged with improper backing.

14. NINE EAGLES DRIVE AT RACETRACK ROAD, SEPT. 29, 1:45 A.M.: A westbound 1999 Volkswagen was driving over the speed limit when it tried to pass another vehicle and crossed the double yellow line. The vehicle lost control, went off the road, hit a wooden utility pole and rolled over onto its roof. The driver, 30-year-old man, was taken to St. Joseph's Hospital.

15. SOUTH VILLAGE DRIVE AT PALMWOOD LANE, SEPT 29, 9:21 A.M.: A northbound 2001 Chevrolet crossed two lanes of traffic into the path of an eastbound 1997 Mazda. A passenger in the Mazda, a 41-year-old woman, injured her left hand.

16. FLETCHER AVENUE AT NEBRASKA AVENUE, SEPT. 29, 12:20 P.M.: A westbound 2001 Dodge van ran a red light and crossed the intersection into the path of a northbound 2000 Chevrolet. The Dodge driver, a 41-year-old man, was charged with disregarding a traffic signal and failure to carry registration.

17. DALE MABRY HIGHWAY AT VAN DYKE ROAD, SEPT 29, 11:03 P.M.: A southbound 1995 Nissan turned left into the path of an eastbound 1998 Honda. The Nissan driver, a 16-year-old girl, was taken to St. Joseph's Hospital.

18. DALE MABRY HIGHWAY AT FLETCHER AVENUE, SEPT. 30, 7:09 A.M.: The driver of a northbound 1987 Pontiac fell asleep, veered right and hit a light pole head-on. The driver, a 28-year-old man, was charged with careless driving and no proof of insurance.

19. FLETCHER AVENUE AT 23RD STREET, SEPT. 30, 2:54 P.M.: An eastbound 1995 Ford rear-ended a 1994 Chevrolet, pushing the Chevrolet into the rear of a 1992 Toyota. The Ford driver, a 21-year-old woman, was charged with careless driving. The Toyota driver, a 36-year-old woman, was taken to University Community Hospital.

20. INTERSTATE 275 AT FLETCHER AVENUE, SEPT. 30, 4:20 P.M.: A northbound 1998 Mitsubishi rear-ended a 1997 Ford truck. The Mitsubishi driver, a 21-year-old man, was charged with careless driving.

21. LYNN TURNER ROAD AT BRADBURY COURT, SEPT. 30, 4:40 P.M.: A southbound 2000 Nissan van rear-ended a 2001 Volkswagen, pushing the Volkswagen into the rear of a 2001 Toyota. The impact pushed the Toyota into the rear of a 1990 Honda.

22. EHRLICH ROAD AT VETERANS EXPRESSWAY, OCT. 1, 6:30 A.M.: A westbound 1985 Jeep van rear-ended a 2000 Pontiac. Both drivers got out to assess the damage and left. The Pontiac driver, a 51-year-old woman, said she felt pain later and called in the report. She had the tag number and described the driver as a white female in her 40s with long, light colored hair.

23. TAMPA PALMS BOULEVARD AT BRUCE B. DOWNS BOULEVARD, OCT 1, 9:59 A.M.: A northbound bicycle turned left into the path of an eastbound 2001 Acura. The bicyclist did not see that the Acura had a green traffic signal and was thrown from the bicycle. The bicyclist, a 36-year-old woman, was taken to St. Joseph's Hospital.

24. INTERSTATE 275 AT BEARSS AVENUE, OCT. 1, 10 A.M.: A southbound 2001 Ford rear-ended a 1988 Nissan. The Ford driver, a 51-year-old man, said his foot slipped off the brake. He was charged with careless driving. The Nissan driver, a 58-year-old woman, was taken to University Community Hospital.

25. FLETCHER AVENUE AT NEBRASKA AVENUE, OCT. 1, 2:20 P.M.: A bicyclist was headed south on the sidewalk and crossed against a red light at a crosswalk into the path of a westbound 1996 Kia. The bicyclist, a 23-year-old man, was knocked off the bike and taken to University Community Hospital.

26. NEBRASKA AVENUE AT CRYSTAL LAKE ROAD, OCT. 1, 2:57 P.M.: A northbound 1986 Ford truck rear-ended a 1995 Pontiac van that was stopped for a red light. The impact pushed the Pontiac into the rear of a 1994 Mercury van. The Ford driver, a 31-year-old man, was charged with careless driving. The Pontiac driver, a 37-year-old woman, was taken to St. Joseph's Hospital.

27. FLETCHER AVENUE AT 22ND STREET, OCT. 1, 6:20 P.M.: A westbound 1994 Dodge rear-ended a 1991 Eagle stopped for traffic. The Dodge driver, an 18-year-old woman, was charged with careless driving.

28. 46TH STREET AT FLETCHER AVENUE, OCT. 1, 6:40 P.M.: A northbound 2000 Ford rear ended a 2000 BMW. The Ford driver, a 22-year-old man, said he thought the BMW was going to turn, and tried to stop.

29. BRUCE B. DOWNS BOULEVARD AT UNIVERSITY SQUARE DRIVE, OCT. 1, 8:20 P.M.: A northbound 2001 Chevrolet turned left into the path of a southbound 1994 Saturn. The Chevrolet driver, an 18-year-old woman, was charged with violation of right of way.

30. BRUCE B. DOWNS BOULEVARD AT 132ND DRIVE, OCT. 2, 6:27 A.M.: A 64-year-old man crossed the street headed east, not using a designated crosswalk, and was hit by southbound 2001 Saturn. The Saturn swerved to the left and hit the brakes. The pedestrian was taken to Veteran's Hospital.

31. FLETCHER AVENUE AT NORTH BOULEVARD, OCT. 2, 2:17 P.M.: An eastbound 2001 Ford truck rear-ended a 1999 Mercury van that was stopped for traffic ahead. The Ford driver, a 26-year-old man, was charged with careless driving. The Mercury driver, a 40-year-old woman, was taken to University Community Hospital.

32. GUNN HIGHWAY AT LINEBAUGH AVENUE, OCT. 2, 4:05 P.M.: An eastbound 1993 Ford truck turned left into the path of a westbound 1986 Honda. The Ford driver, a 23-year-old woman, turned left in a through lane and was charged with violation of a traffic control device. A passenger in the Honda, a 30-year-old woman, was taken to University Community Hospital.

33. INTERSTATE 275 AT BEARSS AVENUE, OCT. 3, 7:45 A.M.: A southbound 1994 Ford rear-ended a 1989 Ford. The 1994 Ford driver, a 38-year-old man, was charged with careless driving.

34. GUNN HIGHWAY AT DALE MABRY HIGHWAY, OCT. 3, 12:20 P.M.: A 2001 Infiniti turned left into the path of a southbound 1998 Dodge. The Infiniti driver, a 24-year-old man, said he had a green arrow. The Dodge driver, a 21-year-old man, stated that he had a green light and skidded 63 feet to avoid the collision.

35. SHELDON ROAD AT WATERS AVENUE, OCT. 3, 2:20 P.M.: A northbound 1984 Honda rear-ended a 1994 Pontiac, pushing the Pontiac into the rear of a 1998 Ford van. The impact pushed the Ford van into the rear of a 1992 Ford. The Honda driver, a 17-year-old woman, was charged with improper equipment.

36. HOLLOWAY ROAD AT GUNN HIGHWAY, OCT. 3, 4:05 P.M.: A 13-year-old girl was riding on the hood of a southbound 1991 Isuzu when it swerved to avoid hitting a dog that crossed the road. The girl was thrown off the hood, dragged under the vehicle and run over. The girl was taken to St. Joseph's Hospital with non-life threatening injuries.

37. DALE MABRY HIGHWAY AT LAKEVIEW DRIVE, OCT. 3, 11:15 P.M.: A northbound 1995 Chevrolet truck ran a red light and turned left into the path of a westbound 1995 Oldsmobile. The Chevrolet driver, a 36-year-old man, was charged with running a red light and no proof of insurance. The injured were taken to University Community Hospital Carrollwood.

38. LINEBAUGH AVENUE AT SHELDON ROAD, OCT. 4, 8:24 A.M.: A westbound 1988 Nissan rear-ended a westbound 2000 Ford truck. The Nissan driver, a 30-year-old man, was taken to University Community Hospital Carrollwood and was charged with following too closely, no proof of insurance and failure to carry a drivers license. The Ford driver, a 35-year-old woman, was treated at the scene for a head injury along with a passenger in the Ford, a 5-year-old boy, who complained of chest pain.

39. VAN DYKE ROAD AT GUNN HIGHWAY, OCT. 4, 3:10 P.M.: A westbound 2001 Dodge van rear-ended a 2000 Dodge van that was having mechanical problems. A passenger in the 2000 Dodge, an 85-year-old woman, complained of neck stiffness but refused treatment.

40. DALE MABRY HIGHWAY AT STALL ROAD, OCT. 4, 4:30 P.M.: A northbound 1982 Datsun rear-ended a 1998 Ford that was stopped for traffic. The impact pushed the Ford into the rear of an unknown vehicle, which left the scene. The Datsun driver, a 17-year-old boy, stated that he glanced away for a second. He was charged with careless driving.
