Accident report

1. HANLEY ROAD AT WILSHIRE DRIVE, OCT. 5, 6:25 A.M.: A 21-year-old woman was struck and killed after crossing the street into the path of a southbound 1991 Ford truck. A traffic homicide investigation was made at the scene.

2. DALE MABRY HIGHWAY AT NORTHDALE BOULEVARD, OCT. 5, 8:09 A.M.: A 2000 Hyundai turned left into the path of a southbound 1994 Ford. The Hyundai driver, a 30-year-old man, was charged with failure to yield right of way.

3. COLLIER PARKWAY AT LIVINGSTON AVENUE, OCT. 5, 1 P.M.: An eastbound 1978 Ford motor home lost control, went off the road and hit a guardrail. The driver, a 44-year-old man, was a habitual offender charged with driving with a suspended license.

4. GOLDEN PRAIRIE ROAD AT AUTUMN WOODS AVENUE, OCT. 5, 2:39 P.M.: A southbound 1988 Dodge truck backed into the front end of a northbound 2001 Toyota truck.

5. FLETCHER AVENUE AT BRUCE B. DOWNS BOULEVARD, OCT. 5, 2:40 P.M.: A southbound 1995 Honda pulled out to make a left turn and went into the path of a westbound 1995 Mazda. The Honda driver, a 26-year-old woman, was charged with violation of right of way. The injured were taken to University Community Hospital.

6. CALUSA TRACE BOULEVARD AT PINE ISLE DRIVE, OCT. 5, 3:10 P.M.: A northbound 1995 Ford truck rear-ended a 1997 Lexus as it slowed to make a right turn. The Ford driver, a 17-year-old man, was taken to St. Joseph's Hospital and charged with following too closely.

7. HIGHWOODS PRESERVE PARKWAY AT BRUCE B. DOWNS BOULEVARD, OCT. 5, 9:30 P.M.: A northbound 1993 Jeep truck made a U-turn in a parking space and hit an eastbound 1996 Mitsubishi head-on. The Jeep driver, a 16-year-old boy, was charged with violation of right of way. The injured were taken to University Community Hospital.

8. DALE MABRY HIGHWAY AT CARROLLWOOD SPRINGS BOULEVARD, OCT. 5, 9:35 P.M.: A southbound 1997 Mitsubishi rear-ended a 1994 Acura. The Mitsubishi driver, a 17-year-old boy, was charged with following too closely. A passenger in the Acura, a 16-year-old boy, was taken to University Community Hospital Carrollwood.

9. GARDNER ROAD AT SEAN ROAD, OCT. 5, 11:40 P.M.: An eastbound 1982 Chevrolet van swerved to avoid people in the road, lost control, went off the road and got hung up in a culvert. The vehicle was still running and the driver was unable to get out of the car. The driver, a 46-year-old man, was charged with refusing a breath test.

10. SOUTH VILLAGE DRIVE AT FAIRWAY RUN, OCT. 6, 1:30 A.M.: A 33-year-old man was headed west in a 2000 Pontiac when he said a vehicle pulled in front of him. He swerved, lost control, went over the median, crossed two eastbound lanes, went off the road, hit a tree and went through a reinforced concrete block wall. Occupants of the vehicle were not restrained and open containers were found in the vehicle. A DUI investigation was conducted at St. Joseph's Hospital where the driver and a passenger, a 34-year-old woman, were taken.

11. LINEBAUGH AVENUE AT MONTAGUE STREET, OCT. 6, 10:20 A.M.: A westbound 1989 Toyota ran a stop sign and entered the intersection into the path of a southbound 1998 Mercedes. The Toyota driver, a 19-year-old man, was charged with running a stop sign. A passenger in the Mercedes, a 56-year-old woman, complained of chest pain from the seat belt. She was taken to St. Joseph's Hospital.

12. LIVINGSTON AVENUE AT COLLIER PARKWAY, OCT. 6, 2:29 P.M.: An eastbound 1997 Dodge van entered the intersection to make a left turn and went into the path of a southbound 2001 Ford. The Ford skidded 26 feet to avoid the collision. The Dodge spun clockwise and came to rest facing southeast. Witnesses said that the Ford was traveling at a high rate of speed.

13. GUNN HIGHWAY AT FOREST LANE DRIVE, OCT. 6, 3:35 P.M.: A westbound 1994 Mercury turned left into the path of an eastbound 1995 GMC. Damage was heavy. The Mercury driver, an 82-year-old man, was charged with turning left in front of approaching traffic. The GMC driver, a 17-year-old girl, was taken to St. Joseph's Hospital.

14. 18TH STREET AT BEARSS AVENUE, OCT. 7, 1 A.M.: A southbound 1997 Ford truck swerved, lost control, hit a chain link fence and hit a tree. The driver, a 40-year-old man, said that he was fooling around making the vehicle swerve when he lost control. The injured were taken to University Community Hospital.

15. 22ND STREET AT 143RD AVENUE, OCT. 7, 7:20 P.M.: A 16-year-old girl was headed north on her bicycle when she was hit by an eastbound 1992 Honda making a right turn. The bicycle was not properly illuminated.

16. FIELDSIDE PLACE AT HIDDEN RIVER PARKWAY, OCT. 7, 8:10 P.M.: An unknown vehicle was backing into a parking space and hit a parked 2002 Kia. The unknown vehicle fled the scene.

17. DALE MABRY HIGHWAY AT FLETCHER AVENUE, OCT. 8, 9:40 A.M.: An eastbound 1996 Nissan pulled into the path of a southbound 1996 Ford. The Nissan driver, a 37-year-old woman, was charged with failure to yield right of way.

18. GUNN HIGHWAY AT COMMUNITY CAMPUS DRIVE, OCT. 8, 12:15 P.M.: A southbound 2000 Toyota van pulled out to make a left turn and went into the path of an eastbound 1998 Ford van. The impact caused the Ford to sideswipe a 1994 Pontiac. The Toyota driver, a 47-year-old woman, was charged with failure to yield right of way.

19. BRUCE B. DOWNS BOULEVARD AT 138TH AVENUE, OCT. 8, 12:30 P.M.: An eastbound 1993 Nissan truck pulled out into the path of a southbound 1988 Pontiac. The Nissan driver, a 47-year-old man, was charged with violation of right of way. The Pontiac driver, a 67-year-old man was taken to James A. Hailey VA Hospital.

20. BRUCE B. DOWNS BOULEVARD AT 138TH AVENUE, OCT. 9, 5:20 P.M.: A northbound 1991 Volkswagen rear-ended a 1996 Nissan, pushing the Nissan into the rear of a 1995 Chevrolet. The Volkswagen driver, a 24-year-old woman, was charged with careless driving. The Nissan driver, a 22-year-old woman, was taken to University Community Hospital.

21. HIGHWOODS PRESERVE PARKWAY AT BRUCE B. DOWNS BOULEVARD, OCT. 9, 9 P.M.: An unknown vehicle backed into a westbound 1996 Toyota while stopped for traffic. The unknown vehicle fled the scene.

22. BRUCE B. DOWNS BOULEVARD AT REGENTS PARK DRIVE, OCT. 10, 7:05 A.M.: A northbound 1999 Honda rear-ended a 1987 Volkswagen. The impact pushed the Volkswagen into the rear of a 1990 Subaru, pushing the Subaru into the rear of a 2001 Mitsubishi. The Honda driver, a 17-year-old girl, was charged with following too closely.

23. 42ND STREET AT VANDERBILT DRIVE, OCT. 10, 12 P.M.: An eastbound 1999 Nissan made a right turn and hit a 19-year-old woman on her bicycle as she approached the intersection.

24. FLETCHER AVENUE AT 19TH STREET, OCT. 10, 2:40 P.M.: An eastbound 1995 Dodge rear-ended a 2000 Toyota, pushing the Toyota into the rear of a 1997 Toyota truck. The Dodge driver, a 22-year-old woman, was charged with careless driving. The Toyota driver, a 33-year-old woman, was taken to University Community Hospital.

25. BEARSS AVENUE AT PINECREST ROAD, OCT. 10, 4:40 P.M.: A westbound 1994 Plymouth turned left into the path of an eastbound 1997 Plymouth. Debris and fluids hit a 2002 Honda motorcycle that was stopped in heavy traffic. The 1994 Plymouth driver, a 63-year-old man, was taken to University Community Hospital and charged with disregarding a traffic control device.

26. BRUCE B. DOWNS BOULEVARD AT 131ST AVENUE, OCT. 10, 7:20 P.M.: An eastbound 1994 Chevrolet truck pulled out of a private drive and crossed the southbound lanes to make a left turn into the path of a southbound 1993 Toyota. The Toyota left 110 feet of skid marks before hitting the vehicle and left another 40 feet of skid marks before coming to a rest. The Chevrolet driver, a 22-year-old woman, was taken to University Community Hospital.

27. 15TH STREET AT 131ST STREET, OCT. 11, 8:36 A.M.: A northbound 2000 Mitsubishi crossed the intersection into the path of an eastbound 1993 Nissan. The Mitsubishi driver, a 28-year-old woman, said she had a green light. The Nissan driver, a 22-year-old woman, said she had the green light. There were no witnesses and no citation was issued. The Nissan driver was taken to University Community Hospital.

28. FLETCHER AVENUE AT LIVINGSTON AVENUE, OCT. 11, 12:06 P.M.: An eastbound 1997 Lexus pulled out to make a left turn and into the path of a 1990 Chrysler. The Lexus driver, 53-year-old man, said the left lane traffic was stopped so a driver motioned for him to make the turn. The Chrysler driver, a 34-year-old woman, was taken to University Community Hospital. The Lexus driver was charged with violation of right of way and no proof of insurance.
