1. CAMBRIDGE AVENUE AT ADAMS STREET, OCT. 6, 3:15 A.M.: A northbound 1985 Toyota truck was going 60 mph in a 30 mph zone when it hit a concrete wall, a mailbox and a utility pole, breaking the pole into two pieces. A witness saw the driver get out of the vehicle and flee on foot. A dog tracked down the driver. The driver was bleeding from the head and had a strong odor of alcohol on his breath. The Toyota driver, a 34-year-old man, was charged with leaving the scene with damage and failure to use due care. He was arrested and taken to University Community Hospital and then to Orient Road jail.

2. BRUCE B. DOWNS BOULEVARD AT PEBBLE CREEK DRIVE, OCT. 6, 7 A.M.: A northbound 1997 Dodge rear-ended a 1996 Mitsubishi that was stopped due to a traffic back-up. The Dodge driver, a 17-year-old girl, was charged with careless driving.

3. DALE MABRY HIGHWAY AT FLETCHER AVENUE, OCT. 6, 2:35 P.M.: A southbound 1985 Oldsmobile turned left into a northbound 1992 Lexus. The Oldsmobile driver, 25, was charged with turning left in front of traffic. Her passenger, a 7-year-old girl, was taken to University Community Hospital.

4. DALE MABRY HIGHWAY AT COLBY ROAD, OCT 6, 5:50 P.M.: A southbound 1991 Honda turned left into the path of a northbound 1997 Honda van. The 1991 Honda driver, a 19-year-old man, was charged with violation of right-of-way and having no insurance.

5. INTERSTATE 275 AT FLETCHER AVENUE, OCT. 6, 7:30 P.M.: An unknown westbound vehicle changed lanes into the right side of a 1990 Acura. The Acura spun off the road and hit a fence. The vehicle that changed lanes fled the scene.

6. INTERSTATE 75 AT BRUCE B. DOWNS BOULEVARD, OCT. 6, 8:18 P.M.: A southbound 1995 Nissan changed lanes into the side of a 1999 Cadillac. The Nissan lost control, spun around and hit a concrete barrier wall. The Nissan driver, a 18-year-old woman, was charged with improper lane change.

7. MORRIS BRIDGE ROAD AT HIGH MEADOW ROAD, OCT. 7, 1:53 A.M.: A stolen southbound 1989 Oldsmobile was traveling 80 mph in a 50 mph zone, lost control, skidded across the northbound lane, hit a curve warning sign, continued sliding sideways, overturned onto its roof and burst into flames. The occupants fled the scene.

8. LINEBAUGH AVENUE AT WILSKY BOULEVARD, OCT. 7, 6:15 A.M.: An eastbound 2000 Infiniti rear-ended a 1995 freight truck. The Infiniti driver, a 28-year-old man, was charged with careless driving and taken to St. Joseph's Hospital

9. FLETCHER AVENUE AT NORTH PALM DRIVE, OCT. 7, 9:45 A.M.: A westbound 1994 Ford truck turned left into the path of an eastbound 1991 Chevrolet. The Ford spun into a northbound 1995 Honda and a northbound 1993 Nissan. The Chevrolet hit a traffic control box and knocked out the power to numerous intersections in the area. The Chevrolet driver, a 36-year-old woman, was taken to University Community Hospital. The Ford driver, a 65-year-old man, was charged with violation of right-of-way.

10. DALE MABRY HIGHWAY AT NORTHDALE BOULEVARD, OCT. 7, 12:58 P.M.: A 1990 Oldsmobile swerved to avoid rear-ending an unknown vehicle that stopped suddenly for no reason and instead the Oldsmobile rear-ended a 1998 Dodge. The unknown vehicle fled the scene.

11. WILSKY BOULEVARD AT LINEBAUGH AVENUE, OCT. 7, 2:59 P.M.: A northbound 1993 Mazda turned left into the path of a southbound 1997 Saturn. The Mazda driver, a 20-year-old man, was charged with violation of right-of-way.

12. FLETCHER AVENUE AT LAKE MAGDALENE, OCT. 7, 4:48 P.M.: A southbound 1997 Nissan pulled out into the path of a westbound 1988 Pontiac. The Nissan spun around and hit an eastbound 1993 Chevrolet. The Nissan driver, a 21-year-old man, was charged with failure to yield. The Pontiac driver, 29, and her passenger, a 27-year-old woman, were taken to University Community Hospital.

13. FLETCHER AVENUE AT 42ND STREET, OCT. 8, 5:15 A.M.: An eastbound 1997 Chevrolet rear-endeda 1999 Chevrolet. The 1997 Chevrolet driver, a 19-year-old man, was charged with driving under the influence.

14. NEW TAMPA BOULEVARD AT BRUCE B. DOWNS, OCT. 8, 10:15 A.M.: An eastbound 1999 Mercedes rear-ended a 1991 Chevrolet van. The Mercedes driver, a 71-year-old man, was charged with careless driving.

15. GUNN HIGHWAY AT ANDERSON ROAD, OCT. 8, 10:45 A.M.: A westbound 1997 Lincoln turned left into the path of an eastbound 1999 Plymouth. The Lincoln driver, a 59-year-old woman, was charged with violation of right-of-way. The Plymouth driver, a 38-year-old woman, and a passenger, a 19-year-old woman, were taken to St. Joseph's Hospital.

16. INTERSTATE 275 AT FLETCHER AVENUE, OCT. 8, 2:22 P.M.: An unknown northbound vehicle pulling a small trailer passed a 36-year-old man driving a 1994 Chevrolet truck that was escorting an oversized load. The unknown trailer hit the side of the Chevrolet.

17. LIVINGSTON AVENUE AT COLLIER PARKWAY, OCT. 8, 2:50 P.M.: A 1998 GMC turned left into the path of a 1985 Chevrolet. The GMC driver, a 27-year-old man, was charged with violation of right-of-way.

18. FLORIDA AVENUE AT APRIL LANE, OCT. 8, 3:12 P.M.: A northbound 1987 Toyota went left of center on a private roadway and hit a southbound 1989 Chevrolet head-on. The Toyota driver, a 33-year-old woman, was charged with careless driving. The Chevrolet driver, a 26-year-old man, was charged with no proof of insurance.

19. CLAY MANGUM LANE AT LAKE MAGDALENE BOULEVARD, OCT. 8, 7:50 P.M.: A westbound 1999 Mercury van went off the road into a field, turned right suddenly and overturned. The driver, a 14-year-old boy, was charged with having no valid driver's license and careless driving.

20. DALE MABRY HIGHWAY AT LINEBAUGH AVENUE, OCT. 9, 7:50 A.M.: A northbound 1987 Nissan changed lanes into the path of a 2000 Chevrolet. The Nissan driver, a 22-year-old woman, was charged with improper lane change.

21. FLORIDA AVENUE AT 131ST AVENUE, OCT. 9, 8:38 A.M.: A westbound 1989 Buick turned left into the path of a northbound 1998 Toyota. The Buick driver, a 42-year-old woman, was charged with a right of way violation.

22. BEARSS AVENUE AT 22ND STREET, OCT. 9, 5:38 P.M.: An eastbound 1986 Oldsmobile entered the intersection on a red light and hit a southbound 1997 Chevrolet. The impact pushed the Chevy into a northbound 1997 Dodge waiting to make a left turn. The Oldsmobile driver, 29, said she tried to stop but her brakes failed. She was charged with running a red light.

23. GUNN HIGHWAY AT ANDERSON ROAD, OCT. 9, 7:30 P.M.: An eastbound 1997 Honda turned left into the path of a westbound 1993 Ford. The impact pushed the Honda into a southbound 1997 Kia stopped at the traffic light. The Honda driver, a 30-year-old man, the Ford driver, a 20-year-old man, and the Ford passenger, a 20-year-old woman, were taken to St. Joseph's Hospital. The Honda driver was cited for failing to yield the right of way.

24. COUNTY LINE ROAD AT BRUCE B. DOWNS BOULEVARD, OCT. 10, 6:45 A.M.: A westbound 1999 Jeep turned left into the path of a 1994 Dodge van. The Jeep driver, a 16-year-old boy, was charged with turning left in front of oncoming traffic. The Dodge driver, a 53-year-old woman, was taken to Tampa General Hospital.

25. DALE MABRY HIGHWAY AT LINEBAUGH AVENUE, OCT. 10, 11:49 A.M.: A southbound 2000 Chevrolet rear-ended a 1998 Toyota, causing the Toyota to rear-end a 1991 Chevrolet van. The Chevrolet driver, a 25-year-old woman, was charged with careless driving.

26. NEBRASKA AVENUE AT SUNSET LANE, OCT. 10, 1:25 P.M.: A westbound 1999 Jeep turned left into the path of an eastbound 1994 Dodge. The Jeep driver, a 16-year-old boy, was cited for turning left in front of oncoming traffic. The Dodge driver, a 54-year-old woman, was taken to Tampa General Hospital.

27. DALE MABRY HIGHWAY AT LAMBRIGHT AVENUE, OCT. 10, 5:28 P.M.: A northbound 1999 Nissan rear-ended a 1990 Pontiac. The Nissan driver, a 30-year-old woman, was charged with careless driving.

28. 22ND STREET AT 131ST AVENUE, OCT. 10, 7 P.M.: An unknown vehicle pulling out onto 22nd Street hit a 17-year-old girl riding a bicycle northbound on the west sidewalk. According to the report, the driver of the car offered to call an ambulance, but the girl denied assistance. She was later taken to University Community Hospital.

29. LIVINGSTON AVENUE AT SINCLAIR HILLS ROAD, OCT. 11, 6:25 A.M.: A southbound 1985 Toyota veered into the northbound lane and hit a 1988 pickup truck and a 1999 Nissan. The Toyota driver, a 39-year-old woman, was charged with careless driving and having an expired tag. She was taken to University Community Hospital.

30. FLORIDA AVENUE AT BEARSS AVENUE, OCT. 11, 6:30 A.M.: A southbound 1985 Ford truck changed lanes and hit a 1993 Chevrolet. The Ford driver, a 34-year-old man, was charged with making an improper lane change.

31. FLETCHER AVENUE AT NORTH BOULEVARD, OCT. 11, 8 A.M.: A 1986 Volkswagen pulled out of a driveway into the path of an eastbound 2000 Mitsubishi, which skidded 99 feet before hitting the Volkswagen. The Volkswagen driver, a 50-year-old man, was charged with a right of way violation.

32. FLETCHER AVENUE AT 22ND STREET, OCT. 11, 11:47 A.M.: A westbound 1996 Kia turned left into the path of an eastbound 1997 Saturn. The Kia driver, a 21-year-old woman, was charged with a right of way violation. The Saturn driver, a 25-year-old woman, the Kia passenger, a 20-year-old man, and the Kia driver were taken to University Community Hospital.

33. BRUCE B. DOWNS BOULEVARD AT COMMERCE PARK BOULEVARD, OCT. 11, 6:35 P.M.: A northbound 1998 Mitsubishi rear-ended a 2000 Mitsubishi stopped at a red light. The 1998 Mitsubishi driver, a 19-year-old woman, was charged with careless driving.

34. 131ST AVENUE AT 20TH STREET, OCT. 11, 8:24 P.M.: A southbound 1992 Ford hit a westbound 1995 Chevrolet. The Ford driver, a 21-year-old man, was charged with running a stop sign.

35. DALE MABRY HIGHWAY AT HUDSON LANE, OCT. 12, 3:25 A.M.: A southbound 1996 Ford traveling 75 mph in a 45 mph zone hit a concrete barrier wall, crossed the grass median into northbound lanes and came to a final rest on the east shoulder. The driver ran from the scene.

36. DALE MABRY HIGHWAY AT EHRLICH ROAD, OCT. 12, 8:55 A.M.: A southbound 1996 Jeep rear-ended a 1994 Buick. The Jeep driver, a 37-year-old woman, was charged with careless driving and having no proof of insurance.

37. DALE MABRY HIGHWAY AT FLETCHER AVENUE, OCT. 12, 9 A.M.: A northbound 1990 Mitsubishi truck changing lanes rear-ended a 1996 Honda slowing for traffic. The impact pushed the Mitsubishi into a 1998 Ford truck that was in another lane. The Mitsubishi driver, a 36-year-old, was charged with failure to use due care. The injured were taken to St. Joseph's Hospital.

38. FOWLER AVENUE AT 22ND STREET, OCT. 12, 12:58 P.M.: A southbound 1996 Dodge turning left hit a 30-year-old woman pushing a stroller. The driver, a 39-year-old man, said he did not see the woman due to the sun's glare. She was taken to University Community Hospital. No charges were filed.

39. INTERSTATE 75 AT INTERSTATE 275, OCT. 12, 1:13 P.M.: A northbound 2000 Dodge sport utility vehicle swerved off the road and hit a light pole after an unknown vehicle cut it off. The unknown vehicle left the scene. The Dodge driver, a 47-year-old woman, was unable to give any information on the vehicle.

40. BEARSS AVENUE AT LAKE MAGDALENE BOULEVARD, OCT. 12, 8:25 P.M.: The driver of a westbound 1997 Isuzu Rodeo took a curve at 60 mph, overcorrected and lost control. The vehicle overturned, ejecting the occupants. The driver and the passenger, both 16-year-old boys, were taken to the hospital where they later died.

41. COUNTRYWAY BOULEVARD AT OAKSBURY DRIVE, OCT. 12, 11:15 P.M.: A westbound 2001 Nissan traveling 45 mph on a TECO access road fishtailed, spun around and flipped. The driver, 16, and his passenger, a 15-year-old boy, were taken to St. Joseph's Hospital.

42. FLORIDA AVENUE AT LAKE MAGDALENE BOULEVARD, OCT. 13, 6:15 P.M.: An eastbound 1998 Hyundai entered the intersection into the path of a southbound 1991 Honda, which skidded approximately 45 feet before collision. The Hyundai driver, a 36-year-old man, was charged with a right of way violation.

43. FLETCHER AVENUE AT 23RD STREET, OCT. 14, 12:45 P.M.: A 1997 Mitsubishi rear-ended a 1999 Toyota. The Mitsubishi driver, a 25-year-old man, was charged with careless driving.
